Once a management plan is established, can it be changed due to unforeseen circumstances?

Care plans are created with the intention of adapting the plan to fit your loved one as they transition through life. Change is a constant and the wishes and wants of the client will likely evolve and as unforeseen circumstances come up the plan is adjusted. Everything is designed to be flexible. You can also ask for any changes at any time.

What if I'm not sure what I'll want for the future, and am not sure what to do for advance planning?

Advance planning may give you and your loved one a roadmap for the future, but if you’re not sure of the destination think of us as someone you can ask for directions. This is our community, too, and we know this area like the back of our hands, so even if you’re not sure where you need to go, we can help you figure it out.

Will life care planning involve the whole family, or just our loved one?

This depends on a lot of factors, the most important being what’s best for you and your family. If your intent is to be there every step of the way you’ll absolutely be included, but if you live at a distance, we can be your eyes and ears, and keep you informed every step of the way. Use us as much or as little as you need.
